The reasons for this were (1) the failure of the FDA, the Defense Department, and its contractor, BioPort Corporation, to plan adequately to ensure the production of a consistent supply of the vaccine in accordance with the FDA regulatory process; and (2) the reliance of the Defense Department on a single private supplier of the vaccine with serious financial problems.
