    Body Mass Index and Decline of Cognitive Function

    Background: The association between body mass index (BMI) and cognitive function is a public health issue. This study investigated the relationship between obesity and cognitive impairment which was assessed by the Korean version of the Mini-mental state examination (K-MMSE) among mid- and old-aged people in South Korea. Methods: A cohort of 5,125 adults, age 45 or older with normal cognitive function (K-MMSE≥24) at baseline (2006), was derived from the Korean Longitudinal Study of Aging (KLoSA) 2006~2012. The association between baseline BMI and risk of cognitive impairment was assessed using multiple logistic regression models. We also assessed baseline BMI and change of cognitive function over the 6-year follow-up using multiple linear regressions. Results: During the follow-up, 358 cases of severe cognitive impairment were identified. Those with baseline BMI≥25 kg/m2 than normal-weight (18.5≤BMI<23 kg/m2) were marginally less likely to experience the development of severe cognitive impairment (adjusted odds ratio [aOR] = 0.73, 95% CI = 0.52 to 1.03; Ptrend = 0.03). This relationship was stronger among female (aOR = 0.63, 95% CI = 0.40 to 1.00; Ptrend = 0.01) and participants with low-normal K-MMSE score (MMSE: 24–26) at baseline (aOR = 0.59, 95% CI = 0.35 to 0.98; Ptrend<0.01). In addition, a slower decline of cognitive function was observed in obese individuals than those with normal weight, especially among women and those with low-normal K-MMSE score at baseline. Conclusion: In this nationally representative study, we found that obesity was associated with lower risk of cognitive decline among mid- and old-age population.

    Cardiovascular health status between standard and nonstandard workers in Korea

    Objectives: The effect of employment insecurity on employee health is an important public health issue due to the recent effects of neoliberalism and the global financial crisis (2007–2008) on labor markets. This study aims to evaluate the differences in cardiovascular health status and the use of preventive screening services between standard and nonstandard workers. Methods: Waged employees (N = 5,338) between the ages of 20 and 64 were grouped into standard (full-time, permanent) and nonstandard (part-time, temporary, or daily) employees. Data from the Fourth Korea National Health and Nutrition Examination Survey, 2007–2009, a nationwide representative survey, were examined, including cardiovascular health risk behaviors (tobacco, alcohol, physical inactivity), measured morbidities (blood pressure, blood glucose level, lipid profiles, body mass index), and the use of screening services for hypertension and diabetes mellitus. Results: Female nonstandard employees tended to have higher blood pressure than did female standard employees (adjusted odds ratio, aOR 1.42, 95% confidence interval, CI 1.02 to 1.98). However, nonstandard employees (both men and women) were less likely to use preventive screening services for hypertension (aOR 0.72, 95% CI 0.54 to 0.94 in men; aOR 0.56, 95% CI 0.43 to 0.73 in women) and diabetes (aOR 0.58, 95% CI 0.43 to 0.79 in men; aOR 0.55, 95% CI 0.43 to 0.71 in women). Conclusion: Nonstandard work is associated with the underuse of screening services and poorer cardiovascular health in a specific population. Policies to reduce employment insecurity and encourage nonstandard employees to receive health screening services should be prioritized.

    Association between alcohol drinking behaviour and cognitive function: results from a nationwide longitudinal study of South Korea

    Objectives: This research intends to determine how drinking behaviour, such as episodic heavy drinking, is related to cognitive performance in middle-aged and old-aged people in South Korea. Methods: A cohort data of 5157 adults, age 45 years or older, with normal cognitive function (the Korean version of the Mini-mental state examination (K-MMSE) ≥24) at baseline (2006), was derived from the Korean Longitudinal Study of Aging. Alcohol drinking behaviour was assessed using the CAGE (Cut down, Annoyed, Guilty, Eye-opener) questionnaire. The relationships between baseline drinking behaviour (in 2006) to the extent of cognitive decline (between 2006 and 2012) and development of cognitive impairment (in 2012) were assessed. Results: Individuals with problematic drinking behaviour at baseline experienced a faster decline in cognitive function than those with non-problematic drinking (p<0.05) during 6 years of follow-up, especially among those with relatively lownormal K-MMSE score (24–26) at baseline (p<0.05). Problematic alcohol drinking behaviour was also significantly associated with onset of severe cognitive impairment (SCI) (K-MMSE score ≤17) among those with relatively low-normal K-MMSE score (adjusted OR (aOR)=3.76, 95% CI 1.46 to 9.67). In addition, abstinence, compared with non-problematic drinking, was related to higher risk for developing SCI among men (aOR=1.62, 95% CI 1.09 to 2.39). Conclusions: Our results suggest that those with problematic alcohol drinking behaviour could be at an increased risk of cognitive impairment/decline. While further research will provide stronger evidence, intervention targeting alcohol abuse may play a role in prevention of cognitive impairment.

    Vitamin D Intake and Risk of Skin Cancer in US Women and Men

    Previous studies suggested a protective effect of vitamin D against skin cancer development. However, epidemiologic studies on orally taken vitamin D and risk of skin cancer (basal cell carcinoma [BCC], squamous cell carcinoma [SCC], and melanoma) are few. We prospectively evaluated whether total, dietary and supplemental vitamin D intake were associated with skin cancer risk based on 63,760 women in the Nurses' Health Study (1984–2010) and 41,530 men in the Health Professionals Follow-up Study (1986–2010). Dietary information on vitamin D intake was assessed every 2 to 4 years during the follow-up and cumulative averaged intake was used. We used Cox proportional hazard models to compute the hazard ratios (HR) and 95% confidence intervals (CI). Pooled HR of cohort-specific results were calculated using a random-effects model. During the follow-up, we documented 20,840 BCC, 2,329 SCC and 1,320 melanoma cases. Vitamin D consumption was not associated with the risk of SCC or melanoma but was modestly positively associated with BCC; the pooled HRs of BCC for extreme quintiles of vitamin D intake were 1.10 (95%CI = 1.05–1.15; Ptrend = 0.05) for total vitamin D and 1.13 (95% CI = 1.07 to 1.20; Ptrend <0.01) for dietary vitamin D. Stratified analysis according to sun exposure related factors showed similar results. In conclusion, vitamin D intake was positively associated with risk of BCC, while null associations were found with SCC and melanoma. Our data do not support a beneficial role of orally taken vitamin D on skin cancer carcinogenesis.
