Publication Consumption of Meals Prepared at Home and Risk of Type 2 Diabetes: An Analysis of Two Prospective Cohort Studies

Consuming food prepared out of the home has been linked to poor diet quality, weight gain, and diabetes risk, but whether having meals prepared at home (MPAH) is associated with risk of type 2 diabetes (T2D) remains unknown. Methods and Findings: We followed 58,051 women (from 1986 to 2012) and 41,676 men (from 1986 to 2010) in two prospective cohort studies of health professionals. Frequencies of consuming midday or evening MPAH were assessed at baseline and during follow-up. Incident T2D was identified through self-report and confirmed using a validated supplementary questionnaire. During 2.1 million person-years of follow-up, 9,356 T2D cases were documented. After adjusting for demographic, socioeconomic, and lifestyle factors, hazard ratios (HRs) and 95% confidence intervals (95% CIs) of T2D were 1 (reference), 0.93 (0.88–0.99), 0.96 (0.90–1.03), and 0.86 (0.81–0.91) for those eating 0–6, 7–8, 9–10, and 11–14 MPAH (p-trend < 0.001) per week, respectively. Participants eating 5–7 midday MPAH had 9% lower T2D risk than those with 0–2 midday MPAH, and participants having 5–7 evening MPAH had 15% lower risk than those with 0–2 evening MPAH (both p-trend < 0.001). In the first 8 y of follow-up, women and men who consumed 11–14 MPAH per week had 0.34 kg (95% CI: 0.15–0.53; p < 0.001) and 1.23 kg (95% CI: 0.92–1.54) less weight gain than those with 0–6 MPAH, respectively. Among participants who were nonobese (body mass index [BMI] < 30 kg/m2) at baseline, pooled HR (95% CI) of developing obesity (BMI ≥ 30 kg/m2) was 0.86 (0.82–0.91; p-trend < 0.001) when extreme MPAH groups (11–14 MPAH versus 0–6 MPAH) were compared. When midday and evening MPAH were analyzed separately, HRs comparing extreme groups (5–7 MPAH versus 0–2 MPAH) were 0.93 (95% CI: 0.89–0.97, p-trend = 0.003) for midday MPAH and 0.76 (95% CI: 0.70–0.83; p-trend < 0.001) for evening MPAH. Further adjusting for BMI during follow-up attenuated the association between MPAH and T2D risk: the HR (95% CI) for participants with 11–14 MPAH was 0.95 (0.89–1.01, p-trend = 0.13). The main limitations of our study were that it lacked assessments on individual foods constituting the MPAH and that the findings were limited to health professionals with a relatively homogeneous socioeconomic status. Conclusions: In two large prospective cohort studies, frequent consumption of MPAH is associated with a lower risk of developing T2D, and this association is partly attributable to less weight gain linked with this dining behavior.
Publication Circulating Persistent Organic Pollutants and Body Fat Distribution, Evidence from NHANES 1999-2004
(2015) Zong, Geng; Grandjean, Philippe; Wu, Hongyu; Sun, QiObjective: To evaluate and compare the correlations of various circulating persistent organic pollutants (POPs) with fat mass percentages (FM%) of trunk, leg, and whole body measured by Dual-energy X-ray absorptiometry. Methods: This study included 2358 adults (≥20 years) in the National Health and Nutrition Examination Survey 1999-2004. Partial Pearson correlation coefficients were calculated, after adjusting for major confounders, including age, smoking status, and history of lactation and parity. Wolfe's method was used to compare correlation coefficients derived from the same participants. Results: Twelve POPs showed significantly different correlations with fat depots in trunk and leg regions. β-hexachlorocyclohexane, heptachlorodibenzo-p-dioxin, and octachlorodibenzo-p-dioxin, and polychlorinated biphenyl (PCB)-126 showed stronger positive correlations with trunk FM% than with leg FM%, whereas PCBs with ≥6 chlorines were more inversely correlated with trunk FM% than leg FM%. Age-stratified analysis showed stronger inverse correlations between POPs and trunk FM% mainly in participants <40 years, whereas stronger positive correlations between POPs and trunk FM% were observed in older participants. Conclusions: Stronger associations between POPs and trunk fat as compared to leg fat, possibly indicated a more important role of trunk fat in the pharmacokinetics of POPs, or a stronger effect of POPs, as endocrine disruptors, on trunk fat metabolism.
Publication Prevalence and risk factors of taste and smell impairment in a nationwide representative sample of the US population: a cross-sectional study
(BMJ Publishing Group, 2016) Liu, Gang; Zong, Geng; Doty, Richard L; Sun, QiObjectives: To estimate the prevalence of, and explore potential risk factors for, taste and smell dysfunction in the general population of the USA. Design: A cross-sectional study. Setting: A cross-sectional analysis of data collected in the National Health and Nutrition Examination Survey (NHANES 2013–2014). Participants: A total of 3519 men and women aged 40 years and older who participated in NHANES 2013–2014. Main outcome measures Using the NHANES Pocket Smell Test, smell impairment was defined as failing to correctly identify 6 or more of the 8 odours. Taste impairment was defined as failing to correctly identify quinine or sodium chloride. Results: The estimated prevalence was 13.5% for smell impairment, 17.3% for taste impairment and 2.2% for taste and smell impairment. For smell, but not taste, prevalence estimates increased with age and were higher in men and ethnic minorities. In backward stepwise logistic regression, low educational attainment, low family income and a history of asthma or cancer were independently associated with a higher prevalence of smell impairment, whereas light-to-moderate alcohol consumption (1–3 drinks/day) was associated with a lower prevalence of this condition. After multivariate adjustment, being non-Hispanic Black Americans, consuming ≥4 drinks of alcohol per day and having a history of cardiovascular disease (CVD) were independently associated with a higher prevalence of taste impairment. Conclusions: Based on a nationally representative multistage probability survey among the US population aged 40 years and older, smell and taste dysfunction affected ∼20.5 million (13.5%) and 26.3 million (17.3%) individuals, respectively. Age, gender, ethnicity, educational attainment, family income, light-to-moderate alcohol consumption and history of asthma or cancer were significant risk factors for smell dysfunction, whereas only ethnicity, heavy alcohol consumption and CVD history were associated with a higher prevalence of taste dysfunction.
Publication Lifestyle of women before pregnancy and the risk of offspring obesity during childhood through early adulthood

Whether such a healthy lifestyle exerts an intergenerational effects on child health deserves examination.
Methods: We included 5,701 children (9-14 years old at baseline) of the Growing Up Today Study 2, and their mothers, who are participants in the Nurses' Health Study II. Pre-pregnancy healthy lifestyle was defined as a normal body mass index, no-smoking, physical activity ≥150min/week, and diet in the top 40% of the Alternative Healthy Eating Index–2010. Obesity during childhood and adolescence was defined using the International Obesity Task Force age- and sex-specific cutoffs. Multivariable log-binominal regression models with generalized estimating equations were used to evaluate the association of pre-pregnancy healthy lifestyle and offspring obesity.
Results: We identified 520 (9.1%) offspring who became obese during follow-up. A healthy body weight of mothers and no smoking before pregnancy were significantly associated with a lower risk of obesity among offspring: the relative risks [RRs; 95% confidence intervals (CIs)] were 0.37 (0.31-0.43) and 0.64 (0.49-0.84), respectively. Eating a healthy diet and regular moderate-to-vigorous physical activities were inversely related to offspring obesity risk, but these relations were not statistically significant. Compared to children of mothers who did not meet any low-risk lifestyle factors, offspring of women who adhered to all four healthy lifestyle factors had 75% lower risk of obesity (RR 0.25, 95%CI 0.14-0.43).
Conclusion: Adherence to an overall healthy lifestyle before pregnancy is strongly associated with a low risk of offspring obesity in childhood, adolescence, and early adulthood. These findings highlight the importance of an overall healthy lifestyle before pregnancy as a potential strategy to prevent obesity in future generations.
Publication Lactation history, serum concentrations of persistent organic pollutants, and maternal risk of diabetes
(Elsevier BV, 2016) Zong, Geng; Grandjean, Philippe; Wang, Xiaobin; Sun, QiObjective: Lactation may help curb diabetes risk and is also known as an excretion route for some environmental pollutants. We evaluated associations of lifetime lactation history with serum concentrations of persistent organic pollutants (POPs) in the National Health and Nutrition Examination Survey 1999-2006, and examined whether potentially diabetogenic POPs account for associations between lactation and diabetes. Research Design and Methods: Among 4,479 parous women, breastfeeding history was defined as the number of children breastfed ≥1 month. Diabetes was identified by self-report or hemoglobin A1c >6.5%. Twenty-four POPs were measured in serum among subsamples of 668 to 1,073 participants. Results: Compared with women without lactation history, odds ratios (95% confidence intervals) of having diabetes among those with 1-2 and ≥3 lactation periods were 0.83(0.61, 1.13) and 0.63(0.44, 0.91; P trend=0.03). Lifetime lactation history was inversely associated with serum concentrations of 17 out of the 24 organochlorine pesticides, polychlorinated biphenyl congeners (PCBs), and perfluoroalkyl substances (P trend<0.05). Comparing the ≥3 lactations group with women without a lactation history, the relative reduction of POPs ranged from 12% (PCB-196) to 30% (oxychlordane). The inverse association between lactation and diabetes was slightly attenuated after adjustment for POPs. Age-stratified analyses showed that the inverse association between lactation periods and serum POP concentrations was observed primarily among participants <60 years, whereas age did not significantly modify the association between lactation history and diabetes prevalence.
Publication Individual and Joint Effects of Early-Life Ambient PM2.5 Exposure and Maternal Prepregnancy Obesity on Childhood Overweight or Obesity

Objectives: The present study was conducted to examine the individual and joint effects of ambient PM2.5 exposures and MPBMI on the risk of COWO. Methods: We estimated exposures to ambient PM2.5 in utero and during the first 2 y of life (F2YL), using data from the U.S. Environmental Protection Agency’s (EPA's) Air Quality System matched to residential address, in 1,446 mother–infant pairs who were recruited at birth from 1998 and followed up prospectively through 2012 at the Boston Medical Center in Massachusetts. We quantified the individual and joint effects of PM2.5 exposure with MPBMI on COWO, defined as the child's age- and sex-specific BMI z-score ≥85th percentile at the last well-child care visit between 2 and 9 y of age. Additivity was assessed by estimating the reduced excess risk due to interaction. Results: Comparing the highest and lowest quartiles of PM2.5, the adjusted relative risks (RRs) [95% confidence intervals (CIs)] of COWO were 1.3 (95% CI: 1.1, 1.5), 1.2 (95% CI: 1.0, 1.4), 1.2 (95% CI: 1.0, 1.4), 1.3 (95% CI: 1.1, 1.6), 1.3 (95% CI: 1.1, 1.5) and 1.3 (1.1, 1.5) during preconception; the first, second, and third trimesters; the entire period of pregnancy; and F2YL, respectively. Spline regression showed a dose–response relationship between PM2.5 levels and COWO after a threshold near the median exposure (10.46μg/m3–10.89μg/m3). Compared with their counterparts, children of obese mothers exposed to high levels of PM2.5 had the highest risk of COWO [RR≥2.0, relative excess risk due to interaction (RERI) not significant]. Conclusions: In the present study, we observed that early life exposure to PM2.5 may play an important role in the early life origins of COWO and may increase the risk of COWO in children of mothers who were overweight or obese before pregnancy beyond the risk that can be attributed to MPBMI alone. Our findings emphasize the clinical and public health policy relevance of early life PM2.5 exposure. https://doi.org/10.1289/EHP261
