Publication Chiasmal visual loss after retinal detachment
(Ovid Technologies (Wolters Kluwer Health), 2012) Prasad, Sashank; Golby, AlexandraA 52-year-old man developed severe vision loss in the right eye due to a large retinal detachment. Vitrectomy was performed in that eye, with 15% C3F8 perfluoropropane gas injected into the posterior chamber.1 He transiently regained hand-motion acuity. Posterior chamber paracentesis was performed for elevated intraocular pressure; endophthalmitis was treated with intraocular antibiotics. After 1 week, blurred vision occurred in the left eye, with intact central acuity and a temporal field deficit (figure). CT imaging demonstrated migration of intraocular gas into the optic nerve and chiasm. Chiasmal visual loss is a rare complication of pneumatic retinopexy.
Publication Ataxia induced by a thymic neuroblastoma in the elderly patient

Whereas most data is taken from childhood neuroblastoma, little is known about the characteristics of the disease in the adult and elderly population. There are significant differences between adult and childhood neuroblastoma which are reviewed below. We report a case of a 62-year-old male who presented with neurological symptoms of ataxia and opsoclonus and an anterior mediastinal mass. Ultimately, the patient underwent a resection of the mass and pathologic review identified a thymic neuroblastoma. This is the first case of thymic neuroblastoma associated with symptomatic central nervous system disease; it is presented with an up-to-date review of the previous cases in the field as well with a review of the literature of post adolescent neuroblastoma.
Publication Evaluating more naturalistic outcome measures: A 1-year smartphone study in multiple sclerosis

Methods: Thirty-eight participant pairs (MS and cohabitant) aged 18–55 years participated in the study. Each participant received an Android HTC Sensation 4G smartphone containing a custom application suite of 19 tests capturing participant performance and patient-reported outcomes (PROs). Over 1 year, participants were prompted daily to complete one assigned test. Results: A total of 22 patients with MS and 17 cohabitants completed the entire study. Among patients with MS, low scores on PROs relating to mental and visual function were associated with dropout (p < 0.05). We illustrate several novel features of a smartphone platform. First, fluctuations in MS outcomes (e.g., fatigue) were assessed against an individual's ambient environment by linking responses to meteorological data. Second, both response accuracy and speed for the Ishihara color vision test were captured, highlighting the benefits of both active and passive data collection. Third, a new trait, a person-specific learning curve in neuropsychological testing, was identified using spline analysis. Finally, averaging repeated measures over the study yielded the most robust correlation matrix of the different outcome measures. Conclusions: We report the feasibility of, and barriers to, deploying a smartphone platform to gather useful passive and active performance data at high frequency in an unstructured manner in the field. A smartphone platform may therefore enable large-scale naturalistic studies of patients with MS or other neurologic diseases.
Publication Fatal Eastern Equine Encephalitis in a Patient on Maintenance Rituximab: A Case Report

C.; Santagata, Sandro; Ahmed, Asim; De Girolami, Umberto; Prasad, Sashank; Mukerji, ShibaniAbstract A 63-year-old woman on rituximab maintenance for follicular lymphoma presented with headaches, vomiting, and fever, and was diagnosed with eastern equine encephalomyelitis by cerebrospinal fluid polymerase chain reaction. Eastern equine encephalomyelitis immunoglobulin (Ig)G/IgM remained negative due to rituximab treatment, and magnetic resonance imaging showed minimal abnormalities, making this a diagnostically challenging case. Despite therapy with intravenous Ig, the patient rapidly declined and died on hospital day 12. Autopsy revealed perivascular and parenchymal chronic inflammation, with an absence of B lymphocytes, and virally infected neurons throughout the central nervous system.
